The customer "Cross-Connection Survey" will be mailed with your water bill 8/06/2024. IEPA requires these surveys be completed every two years. You may complete this survey online by clicking here. If you have any questions, please feel free to contact our office M-F 8:00 am to 4:30 pm. (The Commission is contracted out by other towns to process and send out their customer water/sewer bills. This form is intended for only direct retail customers of the Otter Lake Water Commission - your account # begins with a 1xxxxxx-xx).
